(1) Print or download an application form and have it filled out, signed,

firstname.lastname@example.org with your credit information and trade references;
(3)Wait for us to check the credit information and trade references and verify your information.
Please note that an account application may take 3 - 5 business days to complete.
(4) Once verified and authorized, you may order on-line and fax a formal purchase order to us.
We accept the following credit cards: MasterCard, Visa, American Express, and Discover. We do not allow multiple credit card payments for web orders. Split credit card payments are however accepted for orders placed over the phone. Credit card customers with billing addresses within the 50 United States will not be charged until the order is processed for shipping. Credit card authorization for the order total occurs at the time the order is placed. Credit Cards with billing addresses outside the 50 United States will be charged in full at the time of purchase.
